Notice of Privacy Practices

Effective July 19, 2026 · This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who this notice covers

Tera Pareja, Licensed Mental Health Counselor (WA LMHC LH60792032 · OR LPC C10756 · NPI 1609257328), providing telehealth services to residents of Oregon and Washington.

How your health information may be used

• Treatment. To provide your assessment and any resulting documentation, such as an ESA letter.
• Payment. To process the fee for your session.
• Health care operations. To run the practice — for example, record keeping and quality review.
• Landlord verification. With your request for a housing accommodation, a landlord may be told that your letter is genuine and that your provider is licensed. Your diagnosis and records are never shared in a verification.
• As required by law. Including situations involving serious threats to health or safety, abuse reporting obligations, and lawful court orders.

Other uses and disclosures — including most uses of psychotherapy notes and any marketing use — require your written authorization, which you may revoke at any time.

Your rights

• Get a copy of your health records, and request corrections to them.
• Request restrictions on certain uses and disclosures, and request confidential communications (for example, contact only by a specific phone or email).
• Receive an accounting of certain disclosures.
• Receive a paper copy of this notice on request.
• Complain without retaliation if you believe your privacy rights were violated — to the practice using the contact below, or to the U.S. Department of Health and Human Services Office for Civil Rights.

Our duties

We are required by law to keep your protected health information private, provide this notice, follow its current terms, and notify you if a breach compromises your information. Sessions are held over HIPAA-secure telehealth. If this notice changes, the updated version will be posted on this page.
